Welcome New Students and Parents

Student Heath Center is responsible for making sure all students meet the university’s immunization and tuberculosis screening requirements. ALL new students and transfer students must complete the required forms listed below to complete your enrollment at Frostburg State University. *The required records MUST BE scanned and uploaded through the Patient Portal. Our system is all electronic. NO mailed, fax or email copies of immunization records are accepted.*

Note: If you are a new student, you will not have access to this portal until at least 48 hours after your acceptance fee has been paid and processed.

If you are having difficulty locating your immunization and are a resident of Maryland, you can register with Myirmobile to see if your provider has your immunization on record with myirmobile.
